Americans can obtain a codice fiscale at the Italian consulate covering their US residence before moving to Italy, or at the Agenzia delle Entrate in any Italian city after arrival. Getting it before arrival — through the consulate — is strongly preferable because it enables lease signing, banking preparation, and other pre-departure administrative work.
What the codice fiscale is used for
The codice fiscale is required before you can open an Italian bank account, sign a lease agreement, enroll in Italian healthcare, register a mobile phone contract, purchase property, file Italian taxes, or interact with most Italian institutions in any formal capacity. It is structurally required for Italian administrative life.
The codice fiscale is deterministic — calculated from your surname, given name, date of birth, sex, and place of birth using a published algorithm. Every Italian tax authority official and most bank branch staff can verify a codice fiscale visually. There is only one correct number for any given person, and it remains the same for life.

Getting it after arrival — the Agenzia delle Entrate route
If you arrive in Italy without a codice fiscale, you can obtain one at any Agenzia delle Entrate office — Italy's tax authority, with offices in every provincial capital. The process requires your passport and the completed application form. The number can often be issued immediately during the appointment.
The in-Italy route is faster for immediate issuance but requires that you have already arrived — meaning you managed your first days in Italy without a codice fiscale. During this period, some administrative processes may be delayed.
